Chemical Carcinogen Reports

ANNUAL REPORT
Not later than January 1, of each year, all departments using
chemical carcinogens report the following information to Environmental Health
and Safety (EH&S).
- A brief description of the physical characteristics of all regulated
areas assigned to the department including the building address and any
other location information.
- Specify the chemical names and other identifying information of the
chemical carcinogens present in each regulated area.
- Indicate the number of workers in each regulated area during normal
operations.
- Indicate the number of workers in each regulated area during maintenance
activities.
- Specify whether the chemical carcinogen is manufactured, processed,
used, packaged, repacked, released, stored, or otherwise handled in the
regulated area.
REPORTS OF CHANGES
The department submits a written report of any significant change
to annual-report information within 15 calendar days of the change to EH&S.
ACCIDENT REPORTS
The department reports any incident to EH&S which includes
a release of a chemical carcinogen with any risk of exposure to workers,
S80.72.
- Report the incident to the EH&S within 24 hours, specifying known
information including a report on any medical treatment to workers.
- Specify the amount of material released, the amount of time involved,
and an explanation of the procedures used in determining this information.
- Describe the area involved and the extent of known and possible worker
exposure and area contamination.
- Report any medical treatment of workers and any medical surveillance
program implemented.
- Analyze the incident circumstances, evaluate measures taken, and indicate
plans to avoid further similar releases including specific completion dates.
- Include other pertinent information.
