Equipment Damage or Loss

REPORTING LOSSES OR DAMAGE
Lost or damaged University- or federally-owned equipment are reported as follows:
Inventoried Equipment Losses

Notify WSU Police
Immediately report the loss of all inventoried equipment to WSU Police by telephone or in person; telephone 335-8548.
Inventory Control Report
Initiate an Inventory Control Report. See 20.50.
Inventoried Equipment Accidents
Report equipment which is accidentally damaged to the Department of Risk Management by memorandum. The memorandum includes the accident date, time and circumstances and is signed by the department chair or supervisor.
Insured Equipment Losses
Most University equipment is not insured. WSU operating budgets assume most losses. If lost or damaged equipment is insured, departmental personnel coordinate the claims procedures through the Department of Risk Management.
