Cognitive Alterations
Study/Discussion Questions:
1. Identify causes of acute and chronic confusion in the elderly. Discuss nursing
interventions to prevent acute confusion in the elderly.
2. Compare and contrast acute confusion, chronic confusion and depression. What signs and
symptoms might you see in a client who had more than one of these problems concurrently?
What are the key findings that differentiate one condition from the other?
3. Formulate nursing diagnoses for clients experiencing alterations in cognition.
4. State outcomes a client experiencing alteration(s) in cognition.
5. Formulate appropriate nursing interventions for a client experiencing alteration in
cognition. State the rationale for interventions.
6. Use data from the research studies described in class to develop interventions for
clients with acute or chronic confusion. Discuss the implications of interventions
suggested in the video "Hello in There".
7. Evaluate outcomes of interventions for alterations in cognition.
Case: Think about a client you have had contact with who demonstrates
alterations in cognition. List the signs and symptoms and other observations that led you
to the conclusion that they had an alteration in cognition. List the data given to you
from secondary sources concerning the person's cognitive alteration. Write a nursing
diagnosis based on this data. What interventions did you or other health care team members
implement to manage/address the cognitive alteration? What was the client's response to
each of these interventions? How can you use this data to refine your approach to this and
other clients who have similar prob-lems? What questions do you have as a result of this
exercise? Be prepared to share these in class.
