Case History Josie

Case: Josie True is an 80-year-old retired nurse. She lives alone in a senior citizens apart-ment. She does her own food preparation except she receives Meals on Wheels at noontime Monday-Friday. Her granddaughters do her grocery shopping for her once every 2 weeks from a list she provides. Although she is not on a special diet she tries to avoid salt and fat. Her dentures are uncomfortable so she prefers soft foods. Previous attempts to get her to have her dentures adjusted have met with a firm "No" and a request not to bring this up again. Josie has noticed that she does not have as much appetite as she used to and things don't taste as good as they used to either. Her 24 hour diet recall for Tuesday is as follows:

Breakfast: 1 cup decaffeinated coffee
1 slice dry wheat toast soaked in 1/4 c of skim milk

Lunch: (Provided by Meals on Wheels)
8 oz 1% milk
1/2 c mashed potatoes
3 oz baked chicken breast
1/4 c peas and carrots
1/4 c canned peaches
1 cookie
1/2 c green salad without dressing (ate only 1/2)

Dinner: 1 cup Campbell's low salt Tomato soup
4 Salt free crackers
1/2 medium Banana
She says that this is a "usual weekday diet". She also says she drinks 4-6 of glasses of iced tea or diet soda a day. She seldom drinks water.

Evaluate the adequacy of this diet from the standpoint of both calories and nutrients.
Write a 3-part nursing diagnosis. Write a goal and outcomes for the diagnosis.
Plan a teaching session to address the major problems you identified from your analysis of this data.
