Nursing Process Parts 2 and 3
Learning Assignment:
1. Complete Practice
Activities on the web and submit answers to me by 2/4. (You may do this in
pairs). Be sure the names of both persons appears on the message. This activity is
designed to give you an opportunity to practice and receive feedback on elements of
nursing process that students often have difficulty with when writing plans of care in the
clinical area. This activity is worth 1 point (1% of total grade].) Correct answers
will be posted on the web after the due date for the assignment.
2. It is also a very good idea to complete the Practice Sessions in the text as you go along and compare your answers to those given at the back of the book beginning on page 231.
Study/Discussion Questions: Come to class prepared to apply nursing
process information to case studies. Also come with your questions.
1. How might you use desired outcomes as a starting point for identifying a nursing
diagnosis?
2. Write outcome statements that are client-centered, specific, realistic, individualized,
measurable, and time limited.
3. Discuss ways to engage in mutual goal setting with clients. Why is mutual goal setting
important?
4. Differentiate between nursing actions that are nurse initiated (independent) and those
that are physician initiated (collaborative) or multidisciplinary. Identify direct and
indirect nursing interventions.
5. Determine nursing orders (interventions) by answering the three key questions on page
133 in the Alfaro nursing process text. Identify sources you might use to identify theory
and/or research based nursing orders (interventions).
6. Use the NIC (Nursing Interventions Classification) to identify appropriate
interventions for an individual client. Individualize these interventions for a specific
client by describing, who, what, when, and how.
NIC text is available on reserve in the library.
7. Discuss how to use standard care plans and critical pathways in planning
client care.
8. For each nursing intervention identify the rationale for selection including the
scientific basis for desired outcome (consequences), probability of outcome and value of
the outcome to the client.
9. Evaluate the initial plan of care using the criteria on page 147 in the Alfaro nursing
process text.
10. Describe how you will evaluate various client outcomes. Describe how to decide whether
to terminate, continue or modify the plan of care.
11. Apply each of the steps of the nursing process to planning care for a client.
